Who We Are
HARMONY HOSPICE CARE, INC. is a hospice care agency that’s accredited by the CHAP Accreditation and is based in Carson, California. We serve patients living with advanced and terminal illnesses and their families. Our hospice services offer physical, emotional, and spiritual support to our clients for as long as needed.
Through our services, we aim to help patients manage their symptoms and live with comfort and meaning. We work to help our patients and their families create special moments together by supporting all they need, even in the last phases of their life.
HOSPICE vs. PALLIATIVE CARE
Hospice Care focuses on enhancing the quality of life for the patient and loved ones. It offers palliative and comfort care to patients by using the most current methods of pain and symptom management. Hospice Care begins after the treatment of the disease is stopped.
Palliative Care focuses on providing relief from the symptoms and side effects of the disease. Palliative care also treats emotional, social, practical, and spiritual problems that illnesses can bring up. Palliative Care can begin at diagnosis, and at the same time as treatment.
Mission Statement
It is our mission to help our hospice patients and their families live with dignity, comfort, and meaning. Through our comprehensive and personalized services, we aim to support the physical, emotional, spiritual, and social needs of our clients.
Vision Statement
We envision being a reputable, compassionate, and leading provider in the hospice industry. Our goal is geared towards helping our clients live pain-free and as comfortable as possible to promote their quality of life.
WHO CAN REFER A PATIENT?
Anyone can refer a patient to a hospice program. To be admitted, a patient must:
- Agree to the treatment aimed at comfort rather than cure;
- Have an incurable disease resulting in a limited life expectancy, as a certified by a physician.
WHERE IS CARE PROVIDED?
Hospice care is provided wherever the patient calls home, be it at their own house or at the house of a relative or friend, apartment, nursing home, assisted living facility, long term facility, or hospital.
Palliative care is most often received in the home, apartment, or during short-term hospital admission.
HARMONY HOSPICE CARE, INC. goes above and beyond to provide the best care, comfort, and support to patients and families facing life-limiting illness and loss.
